RESPONSIBILITIES:
Provides individual, crisis, group, couples and family psychotherapy remotely to patients ages 5 thru adulthood. Caseload is comprised of multicultural patients with varying DSM V diagnoses and presenting with high risk or low risk symptoms and behaviors.
Develops and implements the treatment plan by providing psychotherapy interventions and/or concrete services such as complex care management, crisis intervention services, and referrals to outside agencies/providers.
Utilizes data from PSYKES Medicaid database and/or evidence-based tools such as the PHQ9, GAD7, CSSRS, etc. to drive clinical interventions.
Leads psychotherapy groups of up to 12 patients/members.
Follows policy and procedure for “Management of High Risk Patients” on patient caseload. This includes using the Columbia Suicide Severity Screening (CSSRS) and Risk Assessment Tools to continue to assess patient’s risk of harm to self and others.
Confers and consults with members of the treatment team including the psychiatrist, clinical supervisor, and other professional and technical personnel in implementing a multi-disciplinary approach to patient care.
Plans for patient’s discharge from the clinic and/or aftercare.
Maintains appropriate case records on the EHR and prepares other letters and reports as required. Conducts services using collaborative documentation using agency telehealth and EHR systems.
Participates in the collection of data for statistical reports as needed (i.e. – Patient Characteristics Survey, Notification of Hospitalization, ER Visits and 911 Calls).
Reports to the “State Central Registry of Child Abuse & Neglect” all cases of suspected child abuse or neglect.
Reports to the “Justice Center for the Protection of People with Special Needs” all reportable Incidents.Submits Incident Reports to the Clinical Director for all internal and external incidents.
Maintains consistent communication with the treatment team, supervisor, leadership team and other staff via use of technology (Bleuler email, EHR messages and alerts, Voicemails).
Attends clinical supervision meetings remotely, weekly / as scheduled.
Attends staff meetings, trainings and workshops remotely.
Must complete service documentation by end of every day.
Must meet productivity standards of 35 billable services a week.

REQUIREMENTS:
Must Hold one of the following: NYS Licensed Psychologist, Psychoanalyst, Social Worker (LCSW/ LMSW), Mental Health Counselor (LMHC), Marriage & Family Therapist (LMFT), Creative Arts Therapist (LCAT)
Experienced in working with mental illness in children, adolescents, and the adult population.
Electronic Health Record experience, and experience providing telehealth services.
Proficient computer skills including use of MS Word, MS Outlook, EHR, and Telehealth Platforms. Ability and willingness to work mainly through digital tools.
Bilingual in Spanish; Russian; Urdu/Hindi; Farsi; or Mandarin/Cantonese preferred.
Has experience managing a caseload of 35 or more
SKILLS:
Knowledge of the Office of Mental Health Part 599 regulations, and directives for licensed Article 31 clinics and/or integrated clinics.
Knowledge of and adherence to Bleuler Psychotherapy Center guidelines, policies and procedures.
Ability to provide case management and/or crisis services and psychotherapy to clients in need of mental health services.
Organized and high level of initiative with the ability to work independently.
Ability to perform in a team approach and accomplish multiple assignments.
Able to communicate effectively in writing and verbally, and to listen actively.
Community minded, patient, creative, flexible, compassionate, and culturally sensitive to persons who are diagnosed with a mental illness.
Ability to deal tactfully and diplomatically with other Bleuler Psychotherapy Center employees, Federal, State, and City officials, professional and technical groups, patients, and the general public.
REPORTING:
Reports to the Telehealth Coordinator
Work with Clinical Director and Compliance Coordinators on matters of on matters of documentation, adherence to Bleuler policies and procedures, patient complaints and patient care coordination.
