Position Purpose*
We are looking for applicants interested in helping individuals and families in creating their future by providing Human Services!
At Rocky Mountain Human Services, you can find opportunities to build a career working with a wide range of populations from youth to elderly. Depending on the department, you could be working with individuals who have been diagnosed with intellectual or developmental delay, persons needing long-term care, or veterans.
We are currently hiring across different departments for employees who would work to provide case management support specific to the program a client is receiving services under.

Essential Functions*

- Actively communicate and facilitate transdisciplinary review of service plans with ID/D clients, caregivers, families, stakeholders, advocates and providers.
- Actively communicate with customers and stakeholders.
- Works with clients, families and appropriate community agencies to coordinate and facilitate services for clients.

- Ability to meet/work with clients in a variety of settings.
- Ability to meet/work with staff, stakeholders in a variety of settings.
- Attends staff, team and department meetings.
- Attends in-services, staffing and other meetings with supervisor’s approval. May be appointed to committees.
- Develops and maintains client case records, plans and reports.
- Ability to drive personal or company vehicle
- Lift and/or carry 20 lbs.
- Sit, stand and walk for reasonable periods of time.
- Maintains prompt and regular attendance.
- Performs related work as assigned.
Minimum Qualifications
- Bachelor’s degree in human behavioral sciences field, or expected graduation within 1 month of application, (e.g. Social Work, Psychology, Public/Community Health, Special Education, etc.)
- Or 4 years of prior experience within a Case Management role.
*Preferred Qualifications
*
- Bilingual in Spanish or Russian
- Prior personal or professional experience supporting vulnerable populations such as individuals with an intellectual or developmental delay, children with life-limiting illness, or the elderly, blind, and disabled.
- Prior personal or professional experience supporting Veterans or working with homelessness
- Prior professional experience working with individuals with Severe and Persistent Mental Illness (SPMI)

Driving Requirements*
- Valid drivers license
- Proof of motor vehicle insurance
- Personal vehicle in good operating condition for use during work
- No major violations in the past three years.
- No more than two moving violations in the past three years
- Ability to meet and maintain agency driving requirements and operate agency vehicles
