Job Title: Condition Management Remote Patient Monitoring Specialist – Mount Sinai Health Partners

Within MSHP, the Quality, Pharmacy, Value and Efficiency Team is an integral component of clinical operations led by the Deputy Chief Medical Officer. The team drives the implementation and ongoing improvement of initiatives that enable Mount Sinai to deliver better value to its patients, its customers (i.e., plan sponsors and payers), its providers, and its partners. Role Summary
MSHP seeks a Condition Management Remote Patient Monitoring (RPM) Specialist.
The Condition Management Remote Patient Monitoring Specialist is responsible for engaging patients referred to and enrolled in the remote patient monitoring program for condition management through a data-driven approach for prevention, chronic disease management and medication adherence. The Specialist will work with primarily low-risk patients with support from Patient Engagement Coordinators, Clinical Pharmacists, and other collaborating teams, such as nursing and social work resources, to ensure that patients are enrolled and actively participating in the remote patient program. The Condition Management RPM Specialist develops effective interpersonal relationships with patients and works collaboratively with the interdisciplinary care team as well as pharmacies and community-based resources to improve health outcomes. The Patient Engagement Coordinator reports daily to the Director of Condition Management and frequent engagement with the Executive Director of Ambulatory and Population Health Pharmacy.
Responsibilities
Work to improve patients’ experience before, during and following engagement with the health system to provide care continuity. Interacts with patients and caregivers as appropriate to ensure continuity of care, support adherence to care plans, and identify barriers that prevent adherence to care plans.
Outreach and engage with patients about the remote patient monitoring program with the goal of sharing benefits of the program, obtaining consent for enrollment, and kick starting patient journey for home monitoring of chronic conditions.
Assist clinical pharmacists in engaging patients enrolled in remote patient monitoring for medication adherence, lifestyle modification education, or maintenance reminders in using connected devices such as blood pressure cuffs.
Support patients with barriers to full participation in remote patient monitoring such as initial onboarding and device education, collaboration with vendor for technical support, and strategies to improve device use adherence.
Remain available to answer incoming calls from patients and help coordinate care. Initiate MyChart messages and phone calls to proactively reach out to patients who may present as good candidates for remote patient monitoring program.
Follow the patient’s progress during active enrollment in remote patient monitoring program by identifying patient needs, potential barriers, and initiating communications to ensure that the patient receives all services.
Communicate with physicians referring patients to remote monitoring about the outcome of patient engagement, such as patient decline or enrollment.
Once a week, works outside of usual business hours (ex. 1pm-8pm), and once a month on Saturday (ex. 9am-1pm).
Addresses medication adherence issues in coordination with internal care team members and the patient’s pharmacy.
Assists in medical record chart reviews for the purpose of quality audits, and assists in the collection of patient information for external providers, labs, and hospitals.
Documents all patient encounters as appropriate in hospital documentation system (electronic medical record) and assists with the development of standardized reports on remote patient monitoring engagement, processes, and outcomes of care coordination.
Educates patients regarding how to navigate throughout the health system.
Where deemed needed, may spend time helping support practices with remote patient monitoring workflows and dissemination of best practices.
Performs other duties as required.
Qualifications
Education
Bachelor’s degree preferred but not required, ideally in a health-related profession.
High school diploma or equivalent educational diploma (e.g., a GED or foreign diploma), required.
Certified Pharmacy Technician or Medical Assistant, preferred
Experience
Experience in health care, patient navigation/ engagement, care coordinate or related field preferred
Experience in clinical practice setting preferred
Experience collaborating with various provider types such as pharmacists, social workers, etc. preferred
Experience with medication data, medication fill reports, and fill histories, preferred
Additional Skills and Qualities
Fluency in English; Fluency in a second language such as Spanish, Russian, Mandarin a plus
Intermediate skills using Microsoft Office, especially Microsoft Excel
Competency with electronic medical record and scheduling systems
Effective communicator – excellent written and verbal communication for use with patients, staff, and vendors.
Organized – detail-oriented; consistently meets and tracks timelines and objectives
