Job Description
SUMMARY OF DUTIES AND RESPONSIBILITIES:
NYC Health and Hospitals’ “Health is Housing” team develops and implements strategies to better serve homeless patients through a broad range of housing options and services, including but not limited to medical respite; supportive housing; housing navigation and support. The team works closely with H+H care teams to identify homeless patients; accesses suitable opportunities through strategic partnerships with government agencies, housing developers and nonprofit organizations; and supports patients through their transition to permanent housing.
Under the direction of the Senior Housing Specialist, the Housing Specialist will work with patients experiencing homelessness to achieve their permanent housing goal. Housing Specialists manage a caseload of patients experiencing homelessness; develop housing plans to connect these patients to available housing options; coordinates their housing application process; supports move-ins and keeps the H+H care team informed of progress.
The Housing Specialists also serve as a liaison between managing agents, landlords, supportive housing and shelter providers to identify housing options for eligible homeless patients. The role of the Housing Specialist will require substantial field activity, including travel and site visits within the five boroughs of the City of New York, including H+H facilities, DHS shelters, and residential buildings. The priority project will be the utilization of HUD’s Emergency Housing Vouchers.
General tasks and responsibilities will include:
Coordinates with homeless patients’ health care providers to obtain the necessary intake information in order to be the housing subject matter expert for the patient’s care team. Maintain contact with care team as needed during and after housing placement.
Engages with homeless patients by phone and through field-based outreach activities.
Develops housing plans to facilitate connecting patients to available housing opportunities taking into consideration patient geographic preferences, family composition, household’s health care/clinical needs and eligible housing type (supportive/non-supportive). Collaborates with provider to confirm health care connection post-housing placement.
Presents housing needs of patients to the Housing is Health Leadership for assistance in identifying appropriate resources and housing options.
Assists patients with the process of securing housing, including coordinating and accompanying patients to apartment interviews; completing and submitting apartment applications and obtaining supporting documentation, as well as obtaining various subsidies, as appropriate.
Utilizes web-based computer systems including but limited to NYCHA Portal, HRA CAPS, HOME and H+H internal computer systems.
Participates in intra/inter agencies trainings and meetings as required.
Assists patients with securing applicable move-in assistance; coordinating move-ins and inspection processes with landlords and other relevant stakeholders. This includes HRA broker fee, security voucher and move in expenses.
Assists the household with acquiring the necessary documentation and mitigate issues with credit reports, utility arrears, and unfavorable landlord references as necessary.
Educates patients regarding their rights and responsibilities as tenants.
Maintains accurate and comprehensive case documentation for each patient in assigned caseload and provide reports, as needed.
Performs other related duties, as directed.
Minimum Qualifications
A Baccalaureate Degree from an accredited college and two years of experience in community centered activities in an area related to the duties described on the previous page and above; or,
High School graduation or equivalent and six years of experience in community work or community centered activities in an area related to the duties described on the previous page and above; or,
Education and/or experience which is equivalent to (1) or (2). However, all candidates must have at least one year of experience as described in (1).
Department Preferences
PREFERRED SKILLS:
New York City residency (Brooklyn, Bronx, Manhattan, Queens or Staten Island) preferred.
Professional and/or direct experience handling the housing process and/or housing applications in NYC, including affordable housing, supportive housing, market rate housing and City and Federal rental vouchers.
Professional and/or direct experience working with clients experiencing homelessness, housing insecurity, and entitlements.
Excellent written and oral communication skills.
Proficiency in speaking a language other than English, preferably Spanish, Haitian Creole, Mandarin, Bengali, Russian, Cantonese, French, Arabic, Hindi, Urdu, Albanian, or Korean.
Ability and willingness to travel within the five boroughs of New York City to meet patients at either current shelter residence, H+H facility and accompany apartment viewings.
COMPUTER PROGRAMS/SOFTWARE OPERATED:
Microsoft Office (e.g., Word, Excel, Outlook, PowerPoint).
Proficiency with computers, navigating multiple systems and web-based applications. Training will be provided.
