PAY AND BENEFITS
Annual Pay Range: $48,365.75 – $60,943.64
Hourly Pay Range: $24.802950 – $31.253151
Salary offers will be made within the posted pay range and will be based on a candidate’s experience (paid or unpaid) that is directly relevant to the position.
Bilingual Spanish, Russian or SE Asian/English skills are required for this position. An additional 5% compensation is added to pay.

JOB DETAILS AND QUALIFICATIONS
Clackamas County’s Social Services Division is looking for a bilingual (Spanish, Russian and/or SE Asian language) Human Services Coordinator to provide support to the Family Caregiver Support Program (FCSP), on a limited term basis, to assist with determining program eligibility, develop person-centered service plans, and provide ongoing monitoring and oversight of the service plan to ensure stability and sustainability.
We are looking for an energetic, resourceful, warm and empathic individual who works well independently and as part of a team. The incumbent must have an excellent ability to interact with a diverse client base while maintaining professionalism in a stressful environment.
The Family Caregiver Support Program (FCSP) serves unpaid family members caring for older adults, adults with disabilities, or older adults caring for relative children. The goals of the FCSP are to assist family and informal caregivers to care for their loved ones at home for as long as possible, thereby avoiding or delaying the need for costly institutional care. Additionally, the Clackamas County FCSP strives to provide culturally relevant and linguistically responsive services to consumers at their specific need level; provide accessible services, supports and information in a variety of formats to meet individuals’ diverse linguistic, literacy and communication needs; provide a low-cost and effective intervention, allowing people who wish to stay in their home for as long as they are safely able to do so; and provide connections to additional services such as home repairs, low-interest home repair loans, grant programs and other resources that support self-determination and independent living. Ultimately, services will save community and state money and resources.
The Human Services Coordinator’s role is to determine program eligibility through a comprehensive needs assessment, develop person-centered service plans to meet needs, advocate for and coordinate the provision of needed goods and services, and provide ongoing monitoring and oversight of the service plan to ensure stability and sustainability.

Required Minimum Qualifications/ Transferrable Skills:*
A minimum of three (3) years relevant experience in service coordination and/or providing case management to some or all of the following populations: older adults, adults with disabilities, individuals experiencing mental illness, multigenerational families/households, individuals experiencing houselessness, poverty, and individuals at risk for institutionalization in remaining independent
Qualifying experience can be a combination of paid and/or structured volunteer work; and any combination of relevant paid/unpaid experience and relevant education, that would likely provide the required knowledge and skills, may substitute for up to one (1) year of the required years of experience.
A minimum of two (2) years of relevant experience applying the techniques and principles of the assessment and evaluation (including communicating effectively, maintaining records and maintaining confidentiality) of needs of older adults, disabled adults, individuals with behavioral health needs or individuals with cognitive needs, and develop care plans to support those needs
A minimum of two (2) years of relevant experience with EACH the following:
Applying the principles and techniques of motivational interviewing, trauma-informed care, counseling and triaging crisis’ in client services.
Developing and implementing appropriate and responsive plans for clients with diverse needs and diverse goals.
Interpreting and applying federal, state and local laws, rules, regulations and policies.
Maintaining accurate and concise records; preparing and presenting reports, statistics, and related documents.
Establishing and maintaining cooperative working relationships with individuals from diverse groups and backgrounds including clients, families, care providers, community resources, other public and private agencies and County employees.
Managing or coordinating an ongoing caseload of at least 50-75 clients.
Bilingual skills (Spanish/English, Russian/English or SE Asian language/English)

TYPICAL TASKS
Ongoing service coordination or a caseload of consumers served by the Family Caregiver Support Program; annual assessments; ongoing monitoring and follow up; ongoing provision of emotional support; crises intervention service coordination; ongoing information, assistance, referral and advocacy.
Coordinates and assists in planning the activities of the Family Caregiver Support Program; develops and implements work plans to accomplish program goals; allocates other program resources.
Monitors program statistics and client satisfaction measurements; recommends changes for improvements in program services; analyzes and recommends solutions to unusual technical, procedural and resource problems; analyzes community resources, program and client needs, to enhance service delivery.
Screens applicants for program eligibility; matches client needs with available community resources and monitors ongoing services to client; resolves problems and serves as advocate for client-, maintains client case files.
Coordinates the activities of the program(s) or project(s) with other human service programs within assigned department and other community and government agencies; develops professional contacts, attends agency and professional meetings and trainings; makes referrals to other programs/agencies; may assist staff within other programs/agencies.
May assist in preparing and updating budgets and grant proposals; monitors program expenditures and compliance to grant conditions; processes bills, statements and vouchers; exercises cost controls; maintains detailed financial records.
Conducts public relations and outreach activities; creates flyers, brochures, newsletter articles, classified advertisements, press releases and correspondence; develops training materials and delivers public presentations and educational workshops; provides program information to the public, community groups and agencies.
May lead the work of assigned program and administrative support staff and volunteers; assists employees with work problems; trains employees; reviews work.

WORK SCHEDULE
This position is included in the County’s alternate workweek program, working 37.5 hours during a standard workweek of Monday through Thursday (Fridays off). Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.
