ESSENTIAL DUTIES AND RESPONSIBILITIES:
Respond to client inquiries about the Family Caregiver Support program.
Maintain contact with prospective clients, educating them about FSCP services and assisting them through the initial TCARE screening process.
Provide information, assistance, and referral to family caregivers enrolled in FCSP.
Complete TCARE assessments and care plans within 30 days of completing a TCARE screening for new clients.
Perform re-screenings of all enrolled clients. Provide family consultations, depending on capacity.
As part of the initial assessment process, meet with both the caregiver and care receiver. Temporarily this is only offered remotely, in the future it may occur at the location most convenient to the client, which will often be in the community.
Monitor the implementation of client care plans, assisting clients as necessary to meet their care plan goals.
Maintain accurate and complete documentation (either paper or electronic files). Record all FCSP client contact in Salesforce and GetCare.
Staff CHAMP Clinic at Valley Medical Center every other week, meeting with caregiver-care receiver dyads in collaboration with Valley Medical Center providers. This may be provided remotely or in-person.
Work closely with the Director to meet monthly screening, care plan, and outreach targets as well as annual screening and care plan contract requirements.
Maintain information on community resources and refer people to outside services as appropriate.
Attend and participate in Older Adult Services (OAS) clinical consultation meetings and OAS staff meetings as scheduled.
QUALIFICATIONS:
A master’s degree in behavioral or health sciences and one year paid social service experience is preferred; a bachelor’s degree in behavioral or health sciences and at least two years paid social service experience or a bachelor’s degree and at least four years paid social service experience will be considered.
One year of experience in geriatrics or services to family caregivers preferred.
Must become a certified TCARE Assessor within 60 days of hire.
Excellent written and verbal skills. Russian language skills are preferred but not required.
Must have knowledge of community resources.
Strong interpersonal skills are required. Strong interview and assessment skills are needed.
Must be able to work effectively and compassionately with clients who are frail or have physical or cognitive disabilities.
Must have a valid Washington State Driver’s License and access to an insured vehicle.
Working knowledge of Microsoft Word and Outlook and ability to learn to use an electronic database are required.

SALARY AND BENEFITS:
The starting pay for this position is $25.10 to $27.89/hour. Jewish Family Service offers a generous benefits package.
