Position Summary:
Assist older adult clients with initial and renewal applications for benefits and entitlements, housing, orders of protection etc., virtually, in clients’ homes, and at various community sites. Provide support as needed to Geriatric Social Workers working on the Elder Abuse Prevention Project.
Principal Responsibilities:
Assist Geriatric clients with applying and recertifying for benefits and entitlements, housing applications
Assist with document collection
Home visits as needed
Accompany clients to appointments regarding their applications to places such as Social Security, HRA, SCRIE, and others as needed and appropriate
Assistance with applications for emergency food cards and/or financial assistance as needed
Assisting with client intake, elder abuse screenings, case notes, and entering clients into AWARDS data base system
Competencies:
Detail oriented and must be able to document all work in client data system in a timely manner
Patience and ability to manage clients with complex and multiple needs
Ability to work autonomously in the field and take initiative to seek out information and resources that meet clients’ needs
Strong interpersonal skills
Skill and Education:
BSW or Bachelors in related field preferred, BA plus experience accepted
Previous experience working with older adults and/or providing case management a plus
Bilingual English/Spanish or English/Russian preferred. -able to read and write in both languages
Good Computer skills
Physical Demands:
Required Constantly: Walking, sitting, grasping, bending, stooping, squatting, computer input, finger dexterity and coordination of hand, eye and foot
Required Frequently: Standing and climbing stairs; carrying laptop & files (approx. 8-10 lbs)
Required Occasionally: Reaching above the shoulder, and lifting 5-25 pounds, pushing and pulling
