Overview
AMITA Health is assembling a team of Community Health Workers who will serve as “connectors”
between our health care clinicians and our diabetes patients. As a member of this Community Health
Worker Team, you will work with our diabetes patients to assess their level of need for assistance for
things like housing, food assistance, transportation solutions, and to provide assistance by connecting
them with community services that can help them resolve those needs. You will provide education to
them on how to access social and community services and how to achieve and maintain their health in a
culturally appropriate way. As part of our patient services team, you will assist with improving the
quality of care these patients receive by aiding communication between them and their doctor. As a
member of the community, you will develop a working knowledge of local community services and help
our patients navigate those services.

Opportunity
This is an opportunity to contribute to a new program and new approach to health care! Funded for
two (2) years, this experience will provide you the opportunity to work alongside and communicate with
healthcare professionals, to work directly with patients, to assist with resolving patients’ social risks, and
to build relationships with community service providers.
Responsibilities
- Assist patients in understanding their diabetes care plans and instructions
- Conduct screenings for social determinants of health
- Document activities, action plans, and results according to policies and procedures
- Effectively work with people (staff, patients, doctors, agencies, etc.) from diverse backgrounds
to reduce cultural and socio-economic barriers between patients and institutions
- Work collaboratively and effectively within a team
- Motivate patients to be active, engaged participants in their health and facilitate effective
communication with their healthcare provider(s)
- Build and maintain positive working relationships with patients, providers, community service
agency representatives, supervisors, external partners, and office staff
- May occasionally conduct community education programs, make home visits to patients, and/or
assist patients with applications for public services and assistance for which they qualify
- Continuously expand knowledge and understanding of the following:
o Risk factors and management of diabetes
o Community resources, services, and programs
o Human relations and the procedures used in dealing with the public as part of a service
or program
o Operation functions, policies, and procedures associated with the department
o Procedures and resources available to handle new, unusual, or different situations
What You Will Do:
Support health plan members by meeting non-clinical care management needs.
Provide members with wellness coaching in person, virtually and by phone
Assist members with applications for public aid, payer specific programs, accessing medication, disability benefits, and other non-clinical issues as needed.
Locate and facilitate access to community resources for members.
Provide documentation support related to care management
What You Will Need:
Licensure / Certification / Registration
BLS Provider credentialed from the American Heart Association (AHA) preferred.
Education:
High school diploma or GED required.

Requirements
- Bi-Lingual Spanish/English
- Associates Degree or Community Health Worker Certification preferred
- Current driver’s license and reliable mode of transportation
- 3-years of related work experience (community services, non-profit, patient services, etc.)
- Solid computer skills (email, typing, Adobe PDF, Microsoft Office Suite-WORD, EXCEL, PPT) and aptitude
to learn proprietary software (AMITA Health Community Resources (AHCR), EMRs, Tableau, etc.)
- Strong communication skills, verbal and written
